I think this is just that the insert button was falling off screen for certain sized displays. I have moved the media point where the dropdown adjustment occurs so it will now position on screen. When go to a topic with "new" it does not always scroll to the new post, but a couple above it.
Had just had this happen with the Gallery topic.

I've pushed a fix that reindexes the recipient arrays after filtering and added an extra safeguard to prevent this type of mismatch in the future

The notification badge issue appears to be unrelated. Worse, it's apparently a logic conflict in 2.x..

In v 2.x, you can't "reply" to a particular message in a topic - the Reply function at the top (or bottom) of the topic is for the general topic, where the individual posts offer a "Quote" (which makes sense) as the reply is going to end up at the end of the reply chain anyway so you know what post is being replied to.
